To apply for membership in the Private Investigators Association of Utah, complete and submit all the requested information below.  All information, documents and images are maintained confidential (except for directory information you provide).

You will need to upload copies (PDF preferred) of the following documents:

• Current UT BCI issued PI license card
• Current UT BCI issued PI Agency License (agencies only)
• Current driver’s license

Additionally, you will need to upload a current “head-shot” photo image according to the following specifications:

• In color
• Taken within the last 6 months
• Plain white or off-white background
• Full-face view directly facing the camera
• Neutral facial expression and both eyes open
• Taken in clothing that you normally wear on a daily basis
• No uniforms except religious clothing that is worn daily
• No hats or head covering unless worn daily for a religious purpose
• Your full face must be visible
• No headphones, wireless hands-free devices, or similar items
• No sunglasses

Pre-payment of membership fees is required and fees will be refunded if your application is not accepted.  PIAU accepts all major credit cards for payment through our secure payment portal (in the Application below).
